Because it was a habit from turning M shutter speed dial of M cameras with both index and thumb. 
It was very common actually. I have seen many acquaintances doing the same.
Like that is more precise, you never go beyond the desired shutter speed.  

Anyway without thumb rest. My forefinger is always ready to shoot. While my thumb is doing all the settings.

Fortunately. the wheels are next to the shutter button. Or might it be by design?.

Like a piano player needing two fingers to hit the right key?  Of course you never go beyond a desired speed,  given a minimum amount of dexterity. Are you looking at the numbers on an M as well? I can count...
