I have the Leica Q (current firmware 3.0) but need to update the firmware to 3.1 to use Leica Foto app on my iPhone 12. I am concerned that doing so will affect my settings. Do I need to save my current profile to the SD card. If so, is that the same formatted SD card that I will use to download the update? I also have been reluctant to take this step when I read some related threads but these are fairly old as most info seems to be on the newer Q2 so perhaps things have improved.

General IT rule: only update if there is a reason to do so. Never update if there is no need.

You could also say: why use effort and money to develop an update if there is not a reason to do so

The reason need not apply to all owners. My position on updates in general, not just cameras, is: Only update if there’s a compelling reason to do so. Keeping up with the Joneses is not a compelling reason.
