It is more like a ring of 7.5mm thickness. I think it's purpose is to let the lenshead rest on the inner "plateau" of the lensmount, you can see here: Welcome, dear visitor! I didn't try but I think without the sleeve the lens wouldn't focus properly, with it you cannot attach it to the 16598 adapter of the bellows. So Leitz should have given the users of the lens or the lenshead some instruction what to do if they want to use it one way or other. If you attach the lenshead via 16598 to the bellows it enables you to reduce the minimum focussing distance from normally 3.5m to approx. 2.5m or somewhat less if you use the bellows' max. extension. (I couldn't measure it precisely). Though the lens can be focussed to infinity with the bellows as well by reducing the extension  Yes, maybe the sleeve is "fine tuned" to effective focal length, to stop the screwing action on the lens mount... maybe the scratched numbers we have observed (59 60 61...) are related to. Unlickily, I have not the sleeve, because my Telyt V3 was originally sold as lensehead only for Televit (it was regularly listed in this version), so with the famed 14138 ring, but nothing more; I think Leitz didn't listed the transformation of it into a "standard" Telyt 280... and thinking of the sleeve, it was a transformation to be carried on at factory.
