I converted my Summicron 50mm R cam to 3 cams to use on my Leicaflex SL2 as well as my R4S, and used the Elmarit Macro mount as you did. I ended up filing the cam a hint at a time (you can take it off easily enough but you can't put it back), and all the holes were there already. I am not very good at any form of precision DIY, so I was very pleasantly surprised at how easy it was. So the addition of the first and second cam is also possible and easy using the Elmarit Macro tube!! Thank you.
