I've read some horror stories about lug nuts coming loose, and now I'm scared to have my Q2 hanging around my neck (even though I have a solid camera strap for which I use very strong thick strap rings). I went biking this weekend with the Q2 slung over my shoulder, and the whole time I was scared that suddenly the lug would come off the camera and that I wouldn't  be able to grab the strap in time before it fell to the ground.

Am I worrying for nothing or is this a real issue?

Well, if you regurlarly check the lugs for any play you would be safe I suppose. The lug will probably get a little loose long before it falls off.

I have quit the rings. The rings may stress the lugs when they bend back and forth due to strap-twisting. Furthermore they do not allow a full swivel movement of the strap/lug-connection.

I use this simple solution instead. It turns and follows instead of making an opposing force. Regular checking tells me that the simple stop-knot (the simplest possible) does not move or loosen. The rope does of course not wear the lug as metal against metal do.
