Does the adapter have the three tiny cut-outs around its circumference? Do you have another original rear M cap? It should have corresponding nibs to fit into the adapter's cut-out. Insert the nibs into the cut-outs and turn....

 

Another way is to press hard against a piece of rubber (old tire, sole of a shoe etc.) and turn. Make sure you are turning in the correct direction, otherwise you will be tightening the adapter even more.... :p

Screw the adaptor onto a lens (preferably your least valuable one just in case) and loosen the rear lens cap by gently turning and pressing towards the adaptor. If the adaptor is now stuck fast on the lens then do the same again until the adaptor just comes loose from the lens and you should have the cap freed from the adaptor and the adaptor freed from the lens. If not, just jiggle it backwards and forwards until they are. This happens to me fairly often.:)

Success! I found a good way through a friend's advice. Leave the rear lens cap + stuck adapter in a freezer overnight. The freezer will contract the metal adapter slightly but leave the plastic lens cap as it is. With a little fiddling the adapter will come out easily.

Even the high-quality LTM>M adapters, like the Cosima Voigtlanders, sometimes get stuck. Attach them carefully and do not over-tighten.

Users of both LTM and M-series cameras may find these hints useful.

When attaching an LTM to M mount adapter to an LTM lens, always make sure that the adapter is securely and tightly attached to the lens. That way, there is no way for the adapter to come off with the rear lens cap. When removing the adapter from the lens, use the camera body and NOT the lens cap....
