Help with identifying Focaslide

Another request for help. I'm trying to determine which Focaslide model I have. It seems like it is either FULDY or OOZAB. 

I also have the magnifier which I think is LVFOO but not sure. 

And one other item. This item appears to be some type of adaptor for focusing for 5cm Summitar, Summar. There is a number "8576" on the side. 

Again, thanks for insights and patience with a new member. I'm sorting through some of my dad's equipment. He loved is Lecia IIIF!

Your Focaslide is FULDY (1938-47), your magnifier is LVFOO and helical focusing mount for Summar and Summitar is ZWTOO-HESUM (1951).
