The Leica manual (for the MP/M-A, but I presume the M6 etc as well) actually says to advance the film once, before then taking up the slack. Not sure how much difference it makes, but that's what I've done since actually reading the manual (!), and I've not had a bad load since (I take no special measures to ensure the film is on the sprockets before closing the back either, because the manual doesn't say that).

The M6 manual suggests to take up slack first, but I suppose there is a small chance you could pull  the leader back into the cassette if it doesn't have a good grip on the sprockets.  I suppose Leica changed their advice for this reason.

That sounds right. I think with the tulip takeup spool there is nothing to actually grip the film when you turn the rewind to take up the slack.
