Even if one doesn't choose to flatter, there is always the element of realism, and truth to the subject. If one sees and talks to a moving person in real life, one doesn't see, or at least concentrate on, the wrinkles, pores, pimples etc. In a still image, although one is actually seeing the objective truth, one is not seeing the subjective truth, so my choice is normally to soften skin in such portraits (especially the neck - who looks at the neck when you talk to someone?), and occasionally remove pimples etc. It's a question of getting the elements that I think should be balanced, in balance in the image. One then has a better chance of noticing the expression and personality. YMMV, of course.
