Regarding production quality, it varies. My only experience is with Blurb which is reasonable but not without fault. Go to the blurb site and download the free software and play with it. I believe they have recently upgraded to allow you prepare in your own software then print to PDF which they will work from. At the end of the day, all these services tend to be 'one off' jobs, or low volume runs, so it will be sort of expensive. Compare it to having all your pics blown up to nice display size and the book will seem 'cheaper' maybe. A lot of the questions you have I think will be personal decisions in the end so just bite the bullet and consider it the cost of learning and enjoying.
