Two layers of standard cloth camera tape (skinny gaffers tape) trimmed to the mount, and the trusty sharpie turned my LTM Voigtlander into a Voigt-o-Cron. The photo of the lens conversion is just not coming through. There are threads indicating difficulty with coding LTM lenses as the M-adapter ring apparently has a cut out where the sensors are- did you have trouble or can you re-post the picture of the lens conversion?
