I use a Pro Photo Clam head Pro-34NS (for up to 33 lb) with a Photo Clam Universal Plate PC-69-up3, mounted on a Gitzo GT1540 I bought in 2007. I couldn't be happier. It is strong, the plate is 69x39mm in size, with a 1/4" screw, and has 15mm strap eyelets on either end. Bubble levels for both horizontal (landscape) and vertical (portrait) position.
