It is indeed Leica factory made. It was made in small numbers for the medical market. I myself have the same item. And recently it was also again sold on Ebay (item number 120241220490).

Luigi,
The complete setting I post the pict is my own, it is for coupling to and endoscope, the front lever (at 6 oclock)is for engaging disengaging the four balls (you can see them, in the front ring) which clamp over the eyepiece of an endoscope, one of this endo borescope I use to control the inside of rifle barrels.

Ciao Luigi,
There is a lot of special made Visoflex in the scientist world;
here is an another one which is equipped with a large ring in the back and this ring has a groove for clamping on an unknow body.
you can see the little red dome, reminiscence from the locking lever for M mount
